In my opinion, her name should be written Adelaïs du Plessis, in French registries only the birth names are used, marriage names are only added in the form "husband of" or "wife of" (and that mostly in burrial registers)
Birth date would be 1027
Adelaïs, Adelisa and Adrelica are just variations of the same forename but not second names; they could be added in the "also known as" field
