I believe this is Simon V de Montfort, 5th Earl of Leicester rather than Simon IV.

One of the perpetual problems with assigning numbers to these people is that there are different systems. In the best of all worlds, we would eliminate the later conventions that tried to impose numbering systems. But, since we'll never convince people to give them up, it's best to take them all with a grain of salt.
